WHO WE ARE

We provide the anticoagulated patient with a focused and coordinated clinical approach that promotes good outcomes and effective resource utilization.

This is a consultative service for adult outpatients of the Calgary Health Region.

Services include:

• Assessment and education of the referred patient

• Anticoagulation management for patients on oral or subcutaneous anticoagulation therapy

• Assistance with coordinating laboratory testing

• Dosing decisions regarding anticoagulation medications

• Ongoing communication with the patient’s primary physician during the period of therapy

SAFETY

All dosing guidelines and protocols are based on the American College of Chest Physicians as published in CHEST 2004 supplement.

We use the Soprano computer system to record and manage patient data. In addition, there is a working paper patient record that is sent to health records for incorporation into the patient’s regional health record upon discharge from AMS.
